By providing instant access to up-to-date patient records, clinicians can reduce duplication, improve efficiency, and strengthen their working relationships, supporting more effective teamwork and better patient care.

More choice and better outcomes for patients Remote access allows the team to provide safe, efficient care at patients’ homes. “People are being seen in the right place, at the right time, by the right person,” says Jan Fennell-Rutherford, Operations Manager. With improved capacity management, LDP offers patients more flexibility in choosing where and when to receive care. “We can book patients into any clinic they wish,” explains Collette. “If they’d rather a clinic closer to work, closer to home, an evening session.” Faster, safer, more effective care These improvements aren’t just benefitting those who already have diabetes. By leveraging data, the team are identifying at-risk patients and making targeted interventions, supporting better health outcomes across Merseyside. This approach has delivered tangible results: hospital referrals have dropped by 50%, and waiting times have been reduced. “It’s improved secondary care’s capacity to see complex patients,” said Jan, with Collette adding, “If you get a referral today, you can be seen next week by a consultant.” Shorter wait times have also reduced complications, enhancing both patient experience and outcomes.
