PCN increases appointment capacity by 10,000 by transforming service model Using Optum technology Folkestone, Hythe and Rural PCN transformed its service model, enabling 10,000 more appointments across 7 practices serving 48,000 patients. Expanding capacity Initially focused on increasing online consultation uptake, the PCN soon identified broader opportunities to enhance efficiency and patient access, leading to a shift toward a centralised hub model. To address increasing patient demand and workforce challenges, the PCN leveraged the Additional Roles Reimbursement Scheme (ARRS) to expand its workforce. Shifting to a centralised hub model, supported by our EMIS Web PCN Hub clinical system, allowed the team to triage patients across their Network into ARRS practitioners. As Operational Development Manager, Kim Lee said, “GPs now only need to see patients they need to see.”

Making joined-up care a reality With EMIS Web PCN Hub, healthcare teams can seamlessly access patient records without switching systems, ensuring all consultations are recorded in the patient’s registered GP system—streamlining admin and enhancing collaboration. Cutting hospital referrals by 50% for patients with diabetes in Liverpool Leveraging our EMIS Web clinical system, the Liverpool Diabetes Partnership (LDP) has transformed diabetes care, cutting hospital referrals by 50% and reducing waiting times. Patients are now seen within a week of referral, thanks to better collaboration across primary, secondary, and acute care through shared patient records. Safer, more efficient, joined-up care Diabetes care often requires the expertise of multiple specialists, making coordination critical. With EMIS Web, LDP has streamlined collaboration and enabled more informed decision-making. “Problem-solving becomes easier because you can see everything, and clinicians don’t have to repeat details,” says Collette Kelly, Diabetes Specialist Nurse at LDP. By providing instant access to up-to-date patient records, clinicians can reduce duplication, improve efficiency, and strengthen their working relationships, supporting more effective teamwork and better patient care.

More choice and better outcomes for patients Remote access allows the team to provide safe, efficient care at patients’ homes. “People are being seen in the right place, at the right time, by the right person,” says Jan Fennell-Rutherford, Operations Manager. With improved capacity management, LDP offers patients more flexibility in choosing where and when to receive care. “We can book patients into any clinic they wish,” explains Collette. “If they’d rather a clinic closer to work, closer to home, an evening session.” Faster, safer, more effective care These improvements aren’t just benefitting those who already have diabetes. By leveraging data, the team are identifying at-risk patients and making targeted interventions, supporting better health outcomes across Merseyside. This approach has delivered tangible results: hospital referrals have dropped by 50%, and waiting times have been reduced. “It’s improved secondary care’s capacity to see complex patients,” said Jan, with Collette adding, “If you get a referral today, you can be seen next week by a consultant.” Shorter wait times have also reduced complications, enhancing both patient experience and outcomes.

Proactive interventions in Lincolnshire reduce emergency admissions by 58%

To address this, Lincolnshire introduced two dedicated HIU leads to provide non clinical, personalised support. By engaging patients in their preferred settings, the team uncovered the root causes of frequent A&E visits and connected them to relevant resources. Working with voluntary organisations, social prescribers, and local councils, the HIU team developed tailored care plans, helping individuals build resilience and access An evaluation showed that investing £80,000 annually in 2 link workers delivered a 3:1 return on investment within 6 months, preventing £230,000 in emergency admissions. This led to a 58% reduction in admissions and a 41% drop in A&E attendance. This success was made possible by Lincolnshire’s linked data model, which enabled system-wide impact measurement. By shifting to proactive care, Lincolnshire ICS is using data to identify at-risk patients, empowering GPs and community teams to intervene earlier, prevent illness, and reduce hospital admissions. community support. Through a strategic partnership with Optum, Lincolnshire Integrated Care Board (ICB) created a robust Population Health Management (PHM) infrastructure to improve decision-making, resource allocation, and intervention impact measurement. A few years on, Lincolnshire now has a person-level linked dataset covering 100% of the population, enabling them to track patient journeys, monitor health outcomes, and analyse social determinants like housing instability and isolation. Using advanced analytics, they can identify health inequalities and proactively support those most at risk. Matt Gaunt, Deputy Chief Executive and Director of Finance at Lincolnshire ICB, highlights the impact: “It’s already clear that having a fully costed, linked data model has been transformative for Lincolnshire, helping us shift resources toward proactive and preventative interventions and challenging everyone to think, act, and work differently.” Reducing emergency care use with proactive, personalised care One early initiative targeted high A&E attendance. The High Intensity Use (HIU) programme identified frequent adult visitors to A&E, many of whom faced non medical challenges like loneliness, poor housing, or financial struggles.

Technology helps NIHR boost clinical trial recruitment in GP practices Clinical trials are essential for medical progress, but many struggle to recruit enough participants. As custodians of extensive, cradle-to-grave patient data, primary care teams have the potential to transform clinical trial recruitment, supporting research that’s more inclusive and representative of our population. Recruit, our award-winning clinical trial recruitment tool, has helped the National Institute for Health and Care Research (NIHR) invite over 560,000 patients to take part in research via local GP practices. Increasing patient participation As part of the NIHR’s Research Delivery Network (RDN), Regional Research Delivery Networks (RRDNs) in Greater Manchester and the West Midlands used Recruit to support their trial recruitment. Integrated with our EMIS-X clinical system, Recruit allows practices to quickly identify and invite patients to trials in just a few clicks, without adding to their workload. In just 6 months, 4 studies saw over 560,000 invitations sent to potentially eligible patients. In a 2024 report, Newmarket Strategy found that 2 in 3 healthcare professionals are more likely to engage in research when it fits within their existing systems.

This insight was evident in Greater Manchester, which saw a 21% increase in patient invitations from practices using Recruit. Improving access for all patients Greater Manchester and the West Midlands RRDNs have also been able to use Recruit as a mechanism to tackle health inequalities, enabling practices serving underrepresented communities to more easily offer research opportunities to their patients. Supporting GP involvement Recruit not only simplifies participation but also allows practices to receive payments for research activities, aligning with NIHR’s primary care strategy. Paula Lock, Business Manager at the Brooke Surgery, Hyde said: “Recruit was a fantastic starting point for our journey into clinical trials. Having research data and searches readily available within our clinical systems made a significant difference.” By making research a seamless part of primary care, Recruit is helping the NHS, NIHR, and GP practices drive vital studies that improve patient care and the future of healthcare.
