Shared Clinical Services Enabling your clinicians to work effectively together to meet patient needs is essential for a successful PCN. In this section, you’ll find out more about our solutions which support collaborative working, promote safety and enable efficiencies. Our solutions are making it easier to share valuable clinical expertise across your local area whilst ensuring patients are cared for by the right teams. 11

A clinical system for your PCN EMIS Web is the most widely used clinical system for primary care in the UK. Created by clinicians, for clinicians, our software is here to help you run your practice efficiently and deliver the best possible care, with patient safety at its core. With EMIS Web PCN Hub, we’re enabling joined up, integrated care with functionality that supports coordinated working across organisations, including EPS, GP Connect appointments and care record sharing, as well as our GP workload tool. Supported by the relevant sharing agreements, PCN Hub is enabling the relevant clinicians in any service to access the patient records they need to deliver accurate and personalised care, and to manage patient needs from within a dedicated hub environment. With a shared appointment book, practices working together can benefit from cross-organisational booking to manage patient needs against local resources. Consultation Write Back, provides a copy of a consultation to be written back into a patient’s registered GP record when they’re seen at another EMIS Web organisation, promoting clinical safety and empowering collaborative working. New GP Connect Messaging functionality also allows flexibility, by providing an unstructured record (pdf) of a consultation provided by a federated practice to be electronically returned to the ‘usual GP’ for easy filing (supporting a mixture of clinical system suppliers). By providing access to shared patient records and truly joined up care via PCN Hub, your teams will be empowered to work flexibly to support one another with services such as extended access clinics. In addition, our inbuilt safeguarding processes ensure that only the clinicians who need to access a patient’s information will be able to, preventing inappropriate or unnecessary access.

Record sharing View GP record and shared record information from any organisation within a PCN, including from the Hub.

Clinically intelligent decision support Powered by shared EMIS Web records to enable prescribing alerts at the point of care in a hub, or custom alerts relevant to the PCN.

Cross org appointments Book from any organisation into another organisation (including non-EMIS via GP Connect) without the need for multiple log-ins.

Shared protocols and templates Fast and consistent data entry that can be tailored around local enhanced services and shared with the PCN.

Centralised clinical services PCN wide services ran via the hub, supported by custom clinical views and data entry templates - e.g., Diabetes service.

Folkestone Hythe & Rural PCN have been using EMIS technology to underpin the transformation of their service delivery model, making available an additional 10,000 appointments across the PCN. Supported by the PCN Hub clinical system, the team introduced a centralised hub for their ARRS practitioners to enable resource and expertise to be shared across their network, and to relieve pressure from their stretched GPs. Single sign on made it easier for these ARRS clinicians to work efficiently, supporting their seven member practices across the local area. Consultation Write Back functionality introduced into PCN Hub also ensured that each consultation was written back into the native GP record in seconds, supporting joined-up, collaborative working. Embracing our APEX analytics solution helped the team to make data-driven decisions, with dashboards that help them to visualise capacity and demand across their PCN.

We’re empowering your PCN to deliver care closer to home with EMIS Mobile – a solution which allows you to securely and quickly access all the core elements of EMIS Web, wherever you are. With EMIS Mobile, clinicians across your PCN can access medical records, take photos, use body maps and refer to shared care plans whilst away from the office. Whether it’s district nurses working in the community, clinical staff in palliative care hospices or GPs providing home visits, EMIS Mobile makes it simple to manage patient needs on the go. Healthcare professionals can capture rich clinical information using EMIS Mobile at any consultation. With the ability to download and save patient information to access without a connection, as well as write back into patient records and sync when connected, we’re enabling your PCN team to provide better and safer decisions at the point of care.

Managing Medications

EPS enabled care Using the Electronic Prescription Service (EPS) gives your PCN the flexibility to manage patient needs across your organisation, no matter where your patients are based. Combined with digital consulting methods such as video or telephone consultation, clinicians can support patients across their locality and send any prescriptions directly to their local pharmacy, saving unnecessary travel time and reducing footfall. Medication review EMIS Elite partner, CSIS offer the first patient-centred clinical medication review solution for patients with multiple conditions, helping to identify potentially harmful medications. Using this solution within your PCN can help enhance clinical safety, in scenarios where multiple prescribers may be caring for a patient, and help to eliminate any potential adverse drug events.

Shared Administrative Services The ability for PCNs to share administrative services and capabilities can have a huge impact on time saving and patient experience. In this section, we’ll share how our solutions are enabling your PCN the flexibility to deliver services in a way that works for you – from centralised administrative functions to multiple teams collaborating together.

Communication Manage campaigns and recall centrally using enterprise reporting and mail merge. Keeping consistency Ensure consistent ways of working using shared searches, reports, protocols and templates. Bulk registration Upload patients from all member practices into the hub, saving time and ensuring shared patient data is easy to access. Cross org appointments Book into the appointment book of any other PCN member practice, or even create a centralised appointment booking desk. Cross org tasks Send a task to a user at any organisation within the PCN. E.g., record a consul tation in extended hours service and send a task back to the registered GP.

Effective triage and referral with Local Services Deploying Local Services across your PCN can help to reduce pressure, free up appointment time and improve patient care, by allowing you to connect patients with local care providers through a process of patient triage and referral. Built using EMIS-X technology and integrated into EMIS Web, Local Services offers a simple, quick and safe route to triage and refer patients into relevant services, such as CPCS (Community Pharmacist Consultation Service). What’s more, Local Services offers a configurable directory of services, meaning your PCN can tailor your offering in line with the needs of your local community and integrate local referral pathways into your way of working.

Hero Health With EMIS Elite Partner Hero Health, we’re making it easy for patients to book appointments directly into PCN services at a time and date that suits them. Hero Health provide an online booking solution that automates appointment scheduling across your PCN clinics. Your team will be able to open and close EMIS allocated appointment slots where patients can book, reschedule and cancel appointments directly into the EMIS Web appointment book. All information is automatically updated in Hero Health without the need for additional data entry. Intelligent integration with EMIS Web helps you to streamline patient scheduling across clinics, helping to reduce lengthy wait times for patients, minimise additional admin pressures on staff and enhance patient access across your PCN. Clinicians can use Hero’s invite-only scheduling tool to alleviate the stresses of mass patient bookings during high volume services - such as vaccines, blood tests and smears. Simply send an appointment invitation to a selected cohort of patients and allow the patient to book a time and date that fits their schedule.

Enabling proactive, preventative care with Enterprise Search & Reports Our Enterprise Search and Reports solution is the fast and efficient way to source, search and report up-to-date information across your entire Primary Care Network. With the ability to conduct centralised real-time searches on the data held by any practices within your sharing agreement, you’ll benefit from an organisation-wide view of the performance across your PCN. This data can prove invaluable in allowing you to measure the success of your initiatives, track progress against key objectives, measure performance around QOF, LESs/DESs, referral activity and immunisations, and even use data to support the reduction of prescribing waste. You can choose from a range of pre-defined, customisable searches, or you can create your own. Configure searches to repeat for regular reports, in order to enhance service planning and commissioning.

Risk Stratification

We are empowering you to proactively manage patients and prevent hospital admissions with Risk Stratification. Our Risk Stratification solution is a fully integrated tool which uses the peer- reviewed and clinically validated QAdmissions algorithm. Searching for 29 different risk factors within the patient’s record, it produces an individual risk score for your patient. This will then support you to case-find patients and create a personalised care plan, preventing the need for the patient to be hospitalised. You will be supported with a suite of EMIS Web decision-making tools, including concepts, protocols, templates, alerts and searches. These are designed to help you proactively manage patients identified as at risk.

As your PCN grows and evolves in its role within an Integrated Care System (ICS) and under local Sustainability and Transformation Plans (STPs), so does the requirement to work with data at scale across a healthcare system. We understand that access to near real-time analytics to inform decision making is crucial to your Primary Care Network. Population Health APEX enables population health analysis to allow PCNs to understand their population needs and understand how patients currently engage with healthcare systems, to design and optimise healthcare interventions.
