Our Digital Front Door comprises of a suite of integrated solutions that enable patients quick and easy access to a range of healthcare services, whilst supporting multi-disciplinary teams to efficiently manage patient workflow. With these solutions, patients can self-serve, access a wide range of clinically authored information and advice, book appointments, access triage, and manage prescription requests. Practices can direct patients to questionnaires for long term condition management plans which they can submit direct to the practice, often avoiding the need for face to face follow up for long term condition checks. Patients can also speak to clinicians over video consultation or download from a suite of clinically authored leaflets and videos to support their condition, bringing care into the home in a safe and convenient way. Our new service, called Local Services connects patients to local care providers through a process of patient triage and referral. Integrated into EMIS Web and PharmOutcomes, GP staff can easily triage and refer patients to relevant services such as CPCS (Community Pharmacist Consultation Service). When creating the referral, there is the ability to search and select a local pharmacy that is convenient for the patient and book them a referral. Local services also allows practices to refer patients directly to a range of local health services. These secure solutions help connect patients directly to the appropriate care, whilst improving patient outcomes.
