Digital Front Door
There is growing demand for fast, convenient access to services, and healthcare is no exception. Our Digital Front Door comprises of a suite of integrated solutions that enable patients quick and easy access to a range of healthcare services, whilst supporting multi-disciplinary teams to efficiently manage patient workflow. With these solutions, patients can self-serve, access a wide range of clinically authored information and advice, book appointments, access triage, and manage prescription requests. Practices can direct patients to questionnaires for long term condition management plans which they can submit direct to the practice, often avoiding the need for face to face follow up for long term condition checks. Patients can also speak to clinicians over video consultation or download from a suite of clinically authored leaflets and videos to support their condition, bringing care into the home in a safe and convenient way. Our new service, called Local Services connects patients to local care providers through a process of patient triage and referral. Integrated into EMIS Web and PharmOutcomes, GP staff can easily triage and refer patients to relevant services such as CPCS (Community Pharmacist Consultation Service). When creating the referral, there is the ability to search and select a local pharmacy that is convenient for the patient and book them a referral. Local services also allows practices to refer patients directly to a range of local health services. These secure solutions help connect patients directly to the appropriate care, whilst improving patient outcomes. Our Digital Front Door solutions, powered by the EMIS Web platform, create synergy to deliver truly integrated, digitally accessed healthcare. Through these combined solutions, you can gain a greater understanding of your population health, surfacing relevant patient information and developing risk stratification to provide high quality and cost-effective care.
Introducing Online Consult
Our digital first patient triage service, Online Consult supports improved practice access and encourages more personalised care delivery, whilst enabling efficiencies and reducing phone traffic. Accessible via a click through link from your practice website, or via Patient Access, Online Consult uses electronic forms with branching logic to allow patients to securely submit details of their condition to your practice for remote triage. Where relevant, Online Consult will also link patients to online self-help resources, reducing unnecessary appointments and empowering patients to manage their own healthcare needs. All forms have been clinically authored and peer reviewed, and sync with QOF and practice reporting requirements.
