Proactive, preventative healthcare is the future for our NHS. Identifying patients who could benefit from medical intervention can help to improve outcomes and relieve pressure from primary care services. Our Pathway solution uses clinical intelligence within our software to identify at-risk patients, and supports healthcare teams to refer these patients onto the appropriate care pathway. In many instances these pathways are managed outside of primary care by specialist, community or other care services. Our Pathway solution enables GP practices to easily share identified patients with these organisations, helping to reduce the burden on primary care and improve outcomes by placing the patient with those best placed to deliver their care.
